I don't know about Sai Baba being an avatar, however the avataaras of shiva
have been described in the shatarudriiya saMhita. I think it's in volume III of
the verse to verse translation by Shastri.
As far as Balaknath or the person being a gaNa, I don't remember seeing
anything about it in the puraaNa either :-).
