The first sloka became well known after Ramanujacharya
incorporated it into his poetic prayer of surrender,
the SaraNAgati gadya. I believe it is originally from
a minor Purana.

The second vAkya is from the bRhadAraNyaka upanishad.
(asato mA sad gamaya, tamaso mA jyotir gamaya...)
