Dear Hariharan,
There are indeed five temples in which Lord Shiva is worshipped in the
form of the five elements (panca-mahabhuta), space, air, fire, water, and
earth. They are:
Chidambaram Temple in Tamil Nadu == space
Kalahasti in Andhra Pradesh == air
Tiruvannamalai in TN == fire
Jambukeshwara Temple in Tirucirapalli, TN == water
Ekamreshwara Temple in Kancipuram, TN == earth
Om shantih
